In 1982, Mr. Kleinman founded The Americare Companies and sought to expand its range of services and to identify new areas to help those in need of care. The Americare Companies consists of four separate organizations: Americare Certified Special Services, Inc.; Americare, Inc.; Americare Therapy Services; and Global Professional Staffing, Inc.     An experienced home care professional, Elly Kleinman has served since 1982 as the President and Chief Executive Officer of The Americare Companies, a group of four companies offering a range of home care and other healthcare-related services. Through The Americare Companies, Mr. Kleinman has assisted thousands of patients throughout New York State. A member of both the Home Care Association of New York State, Inc. and New York State Association of Health Care Providers, Inc., Elly Kleinman explains the four main categories of home care agencies in the state of New York.

    1. Certified Home Health Agencies (CHHAs): These organizations serve patients who need only a short period of care. CHHAs offer a range of services, including nursing, home health aide assistance, physical, occupational, and speech therapy, social work, and nutrition advice.

    2. Long Term Home Health Care Programs (LTHHCPs): These agencies are specifically supported by the federal government and are sometimes referred to as “Nursing Homes Without Walls.” These organizations provide long-term care to patients who are disabled or have chronic illness.

    3. Licensed Home Care Services Agencies (LHCSAs): LHCSAs serve patients with private insurance, government support, or private resources, contracting with local social services departments and nursing programs to offer a wide range of care options. Some LHCSAs serve a specific small population, such as seniors or children, while others provide care to a variety of patients.

    4. Personal Care Programs: These agencies assist patients with daily, non-medical activities, known as “Activities of Daily Living.” These include mobility, toileting, feeding, meal preparation, personal hygiene, housekeeping, and laundry. Personal Care Programs do not provide medical professionals.

Mr. Kleinman stays active in the healthcare community as a member of the New York State Association of Health Care Providers, Inc. and the Home Care Association of New York State, Inc. Here, he outlines a few of the options for patients who require long-term care and assistance.

    Seniors, people with chronic illness, and others who require long-term medical care have a number of options in front of them, depending on their personal preferences, specific needs, and resources to pay for treatment. Many of these patients prefer to remain in their homes if possible, sometimes with assistance from friends, family members, or professionals. In-home assistance from professionals can range in scope from housekeeping and meal prep to in-home nursing and visits from a physician. Some organizations also offer adult day care programs, which provide assistance with daily activities during the day and allow the patient to remain in his or her home during the evenings.

    Some people choose to move in with a family member or have a family member move in with them in order to provide more full-time care. This care structure can also be supplemented by visits from a home care paraprofessional, nurse, therapist, or social worker.

    Assisted living facilities and continuing care retirement communities offer a residential environment where people can receive a range of care options, from minor assistance in the home to fairly comprehensive daily care. These facilities mimic the environments of home, often offering apartments or town homes with a centralized care staff.

    The most intensive level of care is full-time nursing home care, in which patients move into a dedicated nursing facility for 24-hour-a-day care. This can be less expensive than comprehensive in-home care, and offers more intense supervision and assistance than an assisted living facility or retirement community.
